Community Health Worker, Full-Time (Temporary)

HOURS: 9am - 5pm, however shifts may extend into evenings to accomodate patient needs.

The Community Health Worker (CHW) will be responsible for helping Mercy patients and their families navigate and access health services and other resources. As a priority, the CHW will: decrease inappropriate utilization of the Emergency Department through patient education, establishing a Primary Care Physician when necessary, and securing/maintaining health insurance or charity care. ?CHW will assist patients to additional community services and benefits as appropriate.? Community outreach and collaboration will be required. ?The CHW supports Mercy clinicians through an integrated approach to care management and community outreach.?


Because you?re committed to excellence, you understand the importance of being properly prepared for your role at Mercy. That?s why you?ll bring to your role the right set of qualifications:

  • High School Diploma or equivalent

  • Successful completion of a Community Health Worker formal training program such as from a college or other education institution is preferred and Mercy Training

  • Knowledge of local community and familiar with area service agencies

  • Any combination of 3 years health/social services experience and/or education preferred.

  • Knowledge of some medical terminology preferred

  • Basic computer skills

  • Ability to initiate and maintain positive working relationships with Mercy Co-workers and other community organizations

  • Good communication skills

  • Ability and willingness to provide emotional support, encouragement and motivation to patients

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
