_ Lincoln County Nursing and Rehab _is a skilled nursing facility located in Troy, Mo . We are currently accepting applications for our MDS Coordinator position. The ideal candidate will be a Registered Nurse or Licensed Practical Nurse with prior experience as a MDS Coordinator. RN/LPN license must be in good standing with the State Board of Nursing. Knowledge of skilled nursing facilities survey process and regulations would be helpful.
_ The MDS Coordinator _ is responsible for coordinating the completion of the resident assessment in accordance with current Federal and State regulations. Assists with the development of a comprehensive resident assessment and care plan. Participates in resident/family care plan meetings. Reviews and validates reports and ensures appropriate follow-up action is taken.
Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.
